anthropic: add unit and integration tests

- Unit tests for transformation functions (FromMessagesRequest, ToMessagesResponse)
- Unit tests for error handling and edge cases
- Middleware integration tests with httptest
- Fix lint issues (gofmt)
- Fix unused struct fields in StreamConverter
- Add fallback for crypto/rand errors
